Resolution approving the seventh amendment to the contract between the City and County of San Francisco and Heluna Health to provide comprehensive outreach and case management programming to meet the needs of people experiencing homelessness in San Francisco, known as the San Francisco Homeless Outreach Team, to extend the contract agreement term for 19 months from November 30, 2021, for a total term of August 1, 2014, through June 30, 2023, and increasing the contract amount by $12,685,125 for a total contract amount not to exceed $51,819,067.

The Department of Public Health issued a Request for Proposals to provide outreach and case management services for individuals experiencing homelessness, resulting in the selection of Heluna Health.

A contract was executed with Heluna Health for $6,152,039 to provide outreach services, with a term set to end on June 30, 2015.

The Board established the Department of Homelessness and Supportive Housing to manage services for individuals experiencing homelessness, taking over the contract with Heluna Health.

The contract was amended to increase the amount by $13,367,886, extending the term to June 30, 2021, totaling $39,133,942.

The contract was extended for an additional four months, revising the budget to a total of $39,133,942.

The Board approved the seventh amendment to extend the contract term to June 30, 2023, and increase the contract amount by $12,685,125 for a total of $51,819,067.
