How it got here
This resolution allows the Department of Homelessness and Supportive Housing to amend a grant agreement with the San Francisco Health Plan to enhance services in permanent supportive housing, extending the agreement through December 31, 2025, and increasing funding by $2,820,000. This resolution allows the Department of Public Health to accept an additional $2,046,778 grant from the CDC for HIV prevention and surveillance programs, bringing the total grant amount to $12,756,614 for the period from August 2024 to May 2026. It also authorizes the Director of Health to make necessary amendments to the grant agreement without increasing the city's financial obligations.
