Resolution approving an amendment to the agreement between the City, acting by and through the Department of Disability and Aging Services, and San Francisco Health Plan, for CalAIM community supports services, for a term of four years from July 1, 2024, through June 30, 2028, with an anticipated revenue to the City not to exceed $6,039,300 pursuant to Charter, Section 9.118; and authorizing the Executive Director of the Department of Disability and Aging Services to make necessary, non-material changes to the amendment before its execution.
