Resolution approving Amendment No. 3 to the agreement between the City and County of San Francisco, acting by and through the Department of Disability and Aging Services, and San Francisco Health Plan, for enhanced care management fee for services, having anticipated revenue to the City not to exceed $3,944,000 pursuant to Charter, Section 9.118, with no changes to the term of July 1, 2023, through June 30, 2028; and to authorize the Executive Director of the Department of Disability and Aging Services to enter into any amendments or modifications to Amendment No. 3 that the Department determines, in consultation with the City Attorney, that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the Agreement and Amendment No. 3.

An agreement was established for enhanced care management services between the San Francisco Health Plan and the Department of Disability and Aging Services, effective from July 1, 2023, through June 30, 2028.

The first amendment was made to incorporate CalAIM Incentive Payment Program funds into the agreement.

The second amendment was executed to update the regulatory requirements in compliance with DHCS and DMHC for the agreement.

A proposed resolution was submitted to the Board of Supervisors for the third amendment to the agreement, which included anticipated revenue not to exceed $3,944,000.

The Budget and Finance Committee recommended the proposed third amendment to the Board of Supervisors.

The Board of Supervisors adopted the resolution approving Amendment No. 3 to the agreement.

The Mayor approved the resolution for Amendment No. 3 to the agreement.
