Resolution approving Amendment No. 3 to the agreement between the City and County of San Francisco, acting by and through the Department of Disability and Aging Services, and San Francisco Health Plan, for enhanced care management fee for services, having anticipated revenue to the City not to exceed $3,944,000 pursuant to Charter, Section 9.118, with no changes to the term of July 1, 2023, through June 30, 2028; and to authorize the Executive Director of the Department of Disability and Aging Services to enter into any amendments or modifications to Amendment No. 3 that the Department determines, in consultation with the City Attorney, that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the Agreement and Amendment No. 3.
