Resolution approving Amendment No. 4 to the agreement between the City, acting by and through, the Department of Public Health (DPH), and Project Open Hand, to provide HIV/AIDS food and nutrition services, to increase the contract amount by $1,727,928 for a new total not to exceed amount of $20,528,272 with no changes to the term of April 1, 2017, through March 31, 2027; and to authorize DPH to enter into amendments or modifications to the agreement that do not materially increase the obligations or liabilities to the City and are necessary to effectuate the purposes of the agreement or this Resolution.

The Department of Public Health issued RFP 9-2017 for food and nutrition services targeting low-income HIV clients.

DPH entered into an agreement with Project Open Hand for HIV/AIDS food and nutrition services, initially set for a term until March 31, 2021, with a not-to-exceed amount of $6,258,690.

The agreement was amended to extend the term to June 30, 2021, and increase the contract amount to $7,484,530.

The agreement was further amended to extend the term to June 30, 2022, and increase the contract amount to $9,357,705.

The agreement was amended again to extend the term to March 31, 2027, and increase the contract amount to $18,800,344.

The Civil Service Commission approved the original contract under PSC No. 2005-07/08, which allowed for multiple agreements totaling up to $450 million.

The Board of Supervisors approved Amendment No. 4 to increase the contract amount by $1,727,928, bringing the total to $20,528,272, with no changes to the term.
